    How do AI notes fail during charting?

    AI errors fall into two buckets, and each catches you off guard in a different way.

    Omissions happen when you spoke the words, but they vanished from the draft. You read the lot number out loud, but it is not there. The patient mentioned cold sores, but the history line is blank. You said to hold off on lip filler today, yet the text misses it. Ask yourself one simple thing: what did I say in the room that I do not see on this screen?

    Confident insertions are worse. The AI makes up a detail and presents it as fact. It writes that the patient denies a history of keloids, even though you never asked. It adds a two-week follow-up line when you set no date. It writes that you applied ice post-procedure when you never touched an ice pack. These read great. Nothing about the grammar gives them away. You catch them by matching the page against your actual memory, not against what a standard note ought to say.

    Why does a good-looking AI chart note pose a risk?

    Human charts have typos, weird fragments, and messy lines that force your eyes to stop. AI text reads smooth.

    Smooth text makes you skim. Skimming is how a swapped left and right cheek gets signed into a legal chart. Stop reading for quality. Read for match. The test is never whether the prose sounds smart. The test is whether that exact thing happened in your room.

    Start at the plan, because invented details hide there most.

    Then check the procedure lines. Count your units. Look at every injection site. Verify every mention of left or right.

    Next, run a fast scan for words like denies, reports, or no history of. Those are claims about what came out of the patient's mouth. Make sure she actually said them.

    Finish by checking the lot number, the allergies, the GFE reference, and the follow-up date.
