Security & HIPAA Compliance
Last updated September 2026
Your first questions are usually the most important:"Is this tool secure? Will it protect my patients' data? Are we covered from a compliance standpoint?"
These questions are fundamental to running a responsible healthcare practice. And with AI-powered documentation tools, the stakes can feel even higher.
That's why Symbal's AI documentation platform was designed from the start with healthcare privacy, security, and compliance as core principles—so providers can document consultations confidently while staying focused on patient care.
Below is how Symbal approaches security and compliance.
Why Data Privacy Matters for Clinicians
Data privacy isn't an abstract IT concern. It directly impacts patient relationships, regulatory exposure, and operational stability.
When patients share personal information about their health, they trust that it will remain protected. Any indication that their information may not be secure can quickly undermine that trust.
Protecting patient data is essential to modern clinical infrastructure:
Patient Trust
A breach can immediately damage the trust patients place in your practice.
Regulatory Compliance
Violations of HIPAA or other privacy regulations can result in significant penalties and legal exposure.
Operational Continuity
Secure systems ensure your documentation workflows remain reliable and uninterrupted.
As healthcare practices adopt more digital tools, security is often the top concern.
Where Your Clinical Data Goes with Symbal
Every piece of patient information processed by Symbal is protected through multiple layers of security and privacy controls.
Encryption in transit and at rest
Data is encrypted in transit with TLS 1.2 or higher and at rest with AES-256. Audio is decrypted only inside Symbal's processing environment to produce the transcript and the note, and is never stored unencrypted.
Secure Infrastructure
All systems operate within HIPAA-compliant cloud environments designed to safeguard Protected Health Information (PHI).
Business Associate Agreements (BAAs)
Symbal signs a Business Associate Agreement with every practice at signup, and holds BAAs with each vendor that handles PHI: Amazon Web Services (hosting, U.S. Oregon region), Deepgram (transcription), and OpenAI (note generation).
Access Controls
Strict authentication protocols ensure only authorized users can access patient information.
Role-Based Permissions
Organizations can define user permissions and access levels for clinicians, administrators, and staff.
U.S.-Based Data Storage
All patient data is stored securely within U.S.-based infrastructure.
Account security
Every account is protected by authenticated login. Additional authentication options are available on request.
Flexible Retention Controls
Practices control how long recordings and notes are kept and can request deletion at any time. Deleted data is permanently removed, not archived.
Data Ownership and Clinician Control
With Symbal, clinicians and their organizations retain full ownership and control of their data.
Each account operates within a secure private workspace. Providers can:
- Edit notes
- Export documentation
- Delete records
- Manage retention policies
Even if a practice stops using Symbal, all clinical notes remain accessible for export so providers retain full control over their records.
Symbal does not use identifiable patient data to train AI models.
Any improvements to the system are derived only from de-identified or non-PHI data, ensuring strict privacy boundaries are maintained.
AI Security You Can Trust
Symbal's AI platform is designed to understand clinical conversations while maintaining strong privacy safeguards.
Clinical Speech Recognition
Speech recognition built for clinical conversations, provided by Deepgram under a signed BAA, converts the visit into text.
AI-Assisted Summarization
A language model from OpenAI, under a signed BAA, identifies the clinically relevant information and writes the structured note for your review.
Structured Documentation Templates
Notes can follow preferred documentation formats used by clinicians or organizations.
All processing occurs within HIPAA-compliant infrastructure with encrypted data pipelines and PHI isolation.
Patient Consent Practices
Symbal encourages transparent communication between clinicians and patients regarding documentation technology.
While consent requirements may vary depending on jurisdiction, many clinicians choose to obtain verbal or written consent before recording consultations.
California is an all-party consent state, so every person in the room must agree to the recording. Several other states have the same rule. Confirm the rule for your state.
Common approaches include:
- Verbal confirmation at the start of the visit
- Signage indicating documentation technology may be used during consultations
- Consent forms included during intake or onboarding
Clear communication strengthens patient trust and helps practices maintain regulatory compliance.
AI Model Training and Data Protection
Symbal does not train AI models using Protected Health Information.
Any system improvements rely on de-identified data or non-patient datasets designed to improve accuracy while maintaining strict privacy protections.
Patient conversations and clinical notes remain private to the provider and their organization.
Symbal does not share clinic data with external parties for model training.
Compliance Standards
Symbal's infrastructure is designed to meet modern healthcare security standards.
These protections include:
HIPAA Compliance
Symbal is a business associate under HIPAA and the HITECH Act and is directly subject to the Security Rule, the Privacy Rule, and the Breach Notification Rule. There is no government HIPAA certification; Symbal documents its risk analysis and safeguards and shares them with practices on request.
OWASP Security Standards
Development practices follow secure coding standards and regular security testing.
Encryption Standards
Cryptographic protections follow widely accepted federal security guidelines.
Breach notification
If a breach of unsecured PHI occurs, Symbal notifies the affected practice without unreasonable delay and no later than 10 days after discovery, with the information the practice needs to meet its own notification duties.
Symbal's own Business Associate Agreement is Schedule A of the Platform Terms of Use and is accepted at signup. Details are on the BAA page.
Legal Protections and Offboarding
Symbal's legal framework is designed to protect both healthcare organizations and the platform itself.
Data Portability
Clinicians can export their data at any time.
Flexible Termination
Practices may discontinue use of Symbal and retrieve their records without disruption.
Getting Started Securely
Healthcare providers shouldn't have to choose between strong security and efficient documentation.
Symbal is built to provide both.
